Tragedy struck in the industrial town of Nnewi in Nnewi North LGA of Anambra State as a woman, Mrs. Chiamaka Okonkwo (Twin), was accidentally shot and killed by Agunaechemba operatives. The operatives were at the junction trying to manage traffic when one of them mishandled their weapon, causing the shot to go off.
In a video now circulating online, the lifeless body of the woman is seen lying on the ground as passersby express shock and anger. Some are heard urging that she be taken to the hospital, while others question why a gun was even fired when the operatives were only controlling traffic. Her body was eventually moved using a tricycle.

This isn’t the first time such accidents have happened involving Agunaechemba operatives. Many are now calling for better training in weapon handling and for the state government to set up strict disciplinary measures. There are also demands that the government compensate the victim’s family and ensure justice is served.
The Police Public Relations Officer (PPRO) for Anambra State command, Tochukwu Ikenga (SP) has sued for calm noting that the Command is already in contact with the relevant security outfit to identify the suspect and ensure justice is served.
Meanwhile, the Executive Chairman of Nnewi North LGA, Hon Echezona Anazodo has expressed sadness over the development.
He said ,” I am saddened and greatly disturbed about the accidental discharge that occured on Tuesday April 15th 2025 at old Ibeto Bank junction Nnewi which allegedly took a life of one Mrs Chiamaka Okonkwo
“I wish to commiserate with the immediate and extended family over this irrevocable and ugly incident, please accept my deepest condolences.
“As your mayor, I assure your family and Ndi Nnewi that all is being done since yesternight to determine the real cause of such ugly incident and ensure that adequate justice is served
“Under my watch, no individual or group will hide under any umbrella to commit any form of crime in Nnewi and because of that. I pledge that I will handle the situation with every sense of commitment and make the culprit face the full wrath of the law,” he said.
Hon Anazodo however appealed to Ndi Nnewi to remain calm as he pursue this case decisively noting that he remain committed to the welfare and safety of Ndi Nnewi both indigenous and residents alike.
